Choosing the right Skilled Nursing Facility for your loved ones can be a difficult choice. BSJ Care Center is a five-star facility. Our staff sets the standard for excellence in skilled nursing care. BSJ Care Center has two units, known as Avalon and the Bluffs. Avalon houses our Memory Care Unit. Both units have their own lounge, dining area, Tub Room and Nurse’s Station. Our Recreation Therapy and Occupational Therapy Departments are located between Avalon and the Bluffs. Also located between Avalon and the Bluffs is the Chapel and Beauty Shop.

Activities & Events

BSJ Care Center offers both gated and open areas outside for residents to enjoy. We bring in a wide variety of activities for our residents. Including a petting Zoo, visits from the Badger Band, Grinch sightings and more!

We offer a wide variety of events for our residents, including: Catholic Mass, Ecumenical Worship Service, Bingo, Social and Holiday Parties, Oktoberfest Events, Valentine King & Queen, Card and Game Nights and
Trio Bike Rides.
